Personal Training Services Terms

§1. General Information

  1. The Regulations define the rules for using personal training services provided by Aleksandra Nowak PHYSIOCARE, NIP: 6312737564, REGON: 543580876, ul. Mikołowska 9A, 44-100 Gliwice.
  2. The Regulations constitute an agreement between the Service Provider and the Client.
  3. Using the services means accepting the regulations.

§2. Scope of Services

  1. Services include personal training, consultations, and physical activity planning.
  2. Services do not constitute health or medical services.

§3. Conditions of Participation

  1. A condition for participation is submitting a statement about the state of health.
  2. The Client takes part in the training at their own responsibility.

§4. Reservations

  1. Trainings take place by prior appointment.
  2. Registration takes place:
    • directly;
    • by phone;
    • via the website www.physiocare.com.pl.
  3. The Client's lateness does not extend the training time.

§5. Training Packages

  1. Training packages are personal and cannot be resold.
  2. Validity period:
    • 5 trainings – 30 days
    • 10 trainings – 60 days
    • 20 trainings – 90 days
  3. Unused trainings expire.
  4. Reimbursement for a package is possible only before starting to use a given package.

§6. Cancellation of Trainings

  1. Cancellation or change of the agreed date is possible up to 24 hours before the agreed date.
  2. Later cancellation or change of the agreed date is treated as the service being performed.
  3. In emergencies, the Service Provider may change the date after considering the case.

§7. Payments

  1. Payment takes place before the service is performed - payment "in advance".
  2. Payment can be made by cash, payment card, or bank transfer.

§8. Personal Data

  1. Data is processed in accordance with GDPR.

§9. Final Provisions

  1. The Regulations are valid from 02.01.2026.
