Patient Rights Information
Dear Patient!
Applicable regulations impose on us the obligation to inform you about your rights. To fulfill this obligation at "Aleksandra Nowak PHYSIOCARE", we have prepared this general information on patient rights and an extract from the regulations governing patient rights. In the extract, you will find more detailed information about patient rights and the limitations of these rights specified in the regulations. We encourage you to familiarize yourself with both documents.
Remember that observing patient rights is the duty of public authorities competent in the field of health protection, the National Health Fund, entities providing health services, persons practicing a medical profession, and other persons participating in the provision of health services.
The head of the entity providing health services or a doctor authorized by them may limit the exercise of patient rights in the event of an epidemic threat or due to the health safety of patients. In the case of limiting your rights in our office, you will find information in this regard in our office in an easily accessible place.
YOUR BASIC RIGHTS AS A PATIENT ARE:
- I.Right to health protection.
- II.Right to access health care services financed from public funds.
- III.Right to health services meeting the requirements of current medical knowledge.
- IV.Right to demand that the doctor or nurse providing you with health services consult another doctor or nurse. In the case of a doctor, you can also demand that they convene a medical council.
- V.Right to immediate health services due to a threat to health or life.
- VI.In the case of childbirth, the right to obtain health services related to childbirth.
- VII.Right to health services provided with due diligence, in conditions corresponding to professional and sanitary requirements specified in separate regulations and in accordance with the principles of professional ethics.
- VIII.Right to information about your state of health.
- IX.Right to information about the intention of a doctor to withdraw from treatment.
- X.Right to information about patient rights specified in the Act on Patient Rights and the Patient's Rights Ombudsman and in separate regulations, taking into account the limitations of these rights specified in those regulations.
- XI.Right to information about the type and scope of health services provided by the entity providing health services.
- XII.Right to report an adverse reaction to a medicinal product.
- XIII.Right to confidentiality by persons practicing a medical profession of information related to you and obtained in connection with the practice of the medical profession.
- XIV.Right to consent to the provision of specific health services or to refuse such consent after obtaining information about the state of health.
- XV.Right to respect for intimacy and dignity, in particular during the provision of health services.
- XVI.Right to pain management.
- XVII.Right to the presence of a close person/relative during the provision of a health service.
- XVIII.Right to access medical documentation concerning your state of health and health services provided to you.
- XIX.Right to object to an opinion or certificate specified in Art. 2 sec. 1 of the Act of December 5, 1996 on the professions of doctor and dentist, if the opinion or certificate affects the patient's rights or obligations resulting from the provisions of law.
- XX.Right to additional nursing care.
- XXI.If you are a patient of a medical entity performing medical activities of the type of inpatient and 24-hour health services, you have the right:
- 1) to personal, telephone or correspondence contact with other persons;
- 2) to pastoral care;
- 3) to enable you to contact a clergyman of your denomination in the event of a deterioration in health or a threat to life;
- 4) to store valuables in a deposit.
