Professional Practice Organizational Regulations
1. The Professional Practice (hereinafter referred to as: "Practice") operates under the company name: "Aleksandra Nowak PHYSIOCARE" with its registered office at: ul. Mikołowska 9A, 44-100 Gliwice, NIP: 6312737564 and REGON: 543580876.
2. The Practice is entered into the register of professional practices of physiotherapists, register book number: 000000299433, registration authority: National Chamber of Physiotherapists.
3. The Practice conducts medical activity of the type: ambulatory health services.
4. The Practice provides ambulatory health services in the field of physiotherapy, consisting of:
- patient functional diagnostics;
- qualifying, planning, and conducting physical therapy;
- qualifying, planning, and conducting kinesiotherapy;
- qualifying, planning, and conducting massage;
- conducting physioprophylactic activities, consisting of popularizing health-promoting behaviors and shaping and maintaining the fitness and efficiency of people of various ages in order to prevent disability.
5. Health services are provided exclusively for a fee. The amount of fees for health services provided is specified in the price list.
6. The visit time given during registration is the start time of the health service provision. The patient is obliged to appear at the office 10 minutes before the first scheduled visit time.
7. In the event of a patient's late arrival for a visit, it shall be shortened accordingly.
8. The patient may cancel a visit (change its date) 24 hours before the scheduled visit time. In the event of cancellation (change of date) of a visit after this deadline, the Practice is entitled to claim compensation from the patient in the amount equivalent to the visit cancelled (changed) after the deadline. Each case will be considered individually.
9. In order to ensure the safety of patients and Practice employees (including persons cooperating with the Practice), 24-hour monitoring (hereinafter: "Monitoring") of public rooms is conducted in the office, which covers the reception area and the waiting room.
10. Monitoring is carried out using image recording devices (video surveillance). Sound is not recorded as part of Monitoring.
11. Personal data obtained during Monitoring is processed to ensure the safety of patients as well as employees and associates of the Practice.
12. Recordings obtained during Monitoring are stored for a period not exceeding 3 months. After this period, image recordings containing personal data obtained as a result of Monitoring are subject to destruction, unless separate regulations provide otherwise.
13. Recordings recorded during Monitoring may be made available to third entities exclusively on the basis of applicable laws.
14. The Practice informs patients, employees, and associates about conducting Monitoring in a manner consistent with generally applicable law, and in particular by familiarizing them with the appropriate information obligation clause.
